The Heart Failure Clinic at East Jefferson General Hospital is an MPSI practice managed by East Jefferson General Hospital.

The Heart Failure Clinic at East Jefferson General Hospital answers a need within our community for heart failure patients to have ready access to testing, evaluation and clinical support, including follow-up appointments after hospital admission or visit to the emergency department, and ongoing medical management.

Access to Care

The Heart Failure Clinic is dedicated to providing easy referral of heart failure patients for follow-up and/or continued heart failure management.

  • Within 72 hours of hospital discharge
  • Within 24-48 hours of discharge from an Emergency Department

Dr. Sabrina M. White, DNP, MSN, APRN, ACNP, B.C.

Dr. Sabrina White received her Doctorate in Nursing Practice and Masters of Science in Nursing from the University of South Alabama in Mobile, Alabama. She is certified as an Acute Care Nurse Practitioner with a concentration in cardiovascular disease. She was awarded the Florence Nightingale Award for Advance Practice Nursing and two awards in Outstanding Clinical Performance in Medical-Surgical Nursing. She is experienced in both Advanced Cardiopulmonary Life Support and Basic Cardiopulmonary Life Support. In addition to being a cardiovascular nurse for over twenty years, her concentration and focus is heart failure and heart transplantation with publications pertaining to reducing length of stay and readmission rates among heart failure patients. Her first nursing job was at Charity Hospital in 1991.
